Step 1: Schedule your first appointment.

Step 2: Complete new client paperwork
All adults coming to therapy will need to complete their own online registration (both the online registration and the consent packet). Your forms, once submitted, will be sent to your therapist automatically via HIPAA protected 3rd party providers with whom we have signed a Business Associates Agreement. If you have concerns about submitting this form electronically, please contact your therapist to receive a paper copy.
****Under the No Surprises Health Care Act, all self-pay patients are entitled to a Good Faith Estimate (GFE) of potential treatment costs. If you are not provided with this estimate prior to receiving services, you have the right to ask your provider for one. Additionally, should you be charged in excess of this estimate, you have the right to appeal these charges to both the provider and the Department of Health and Human Services. When you complete our new client registration form, you will be provided with an initial GFE, if you are a self-pay client. Any questions regarding this estimate can be directed to your therapist.
Step 3: Logging on
All of our services are delivered via tele-health.
We currently use Zoom for virtual appointments. You therapist will send you the meeting link prior to your 1st appointment. We recommend a private place with a strong wifi. Plan on using something other than a cell phone. You will need a large stationary screen, the bigger the better. Give yourself time to transition into and out of the appointment-just like you would in an office setting.
If you aren’t familiar with Zoom, we recommend spending time with it before your first appointment. You may have to troubleshoot.
